Care Management Services at The Village at Duxbury
Our Care Management Department is dedicated to keeping our residents healthy and providing them support when needed. The goal is to assist residents with planning for their future care needs and to maximize independence and safety while maintaining their quality of living.
On-site professional nursing staff and resident assistants are available 24-hours-per-day. The Care Management Director provides knowledge, guidance and expertise to determine the scope of care an individual needs to ensure their personal preferences. The Director consults with residents and their family members and works with discharge planners to help transition a resident back to independent living after a hospital, skilled nursing or rehabilitation stay. Our nurses coordinate the services provided to our residents by in-home health agencies and private companions.

- Updating resident emergency medical information
- Assessment of resident illness
- Assistance with medications
- Wound care simple dressings
- Administration of B12 injections
- Post-hospitalization visits
- Coordination of therapy
- Assistance with acquisition of adaptive equipment
- Overnight checks
- Social worker counseling
- Geriatric and podiatry visits
- Physician, dental, eye and pharmacy referrals
- Individual care management
- Assistance with placement in assisted living and skilled nursing facilities
